Just be descretely technical. I get that it is a nads reference, what I don't get is why you all think it's Riff subtley coming on to Gwenn.

"blue balls" is a comment some men make when a woman teases them sexually but then *shiftyeyes* er, how to put this... the man doesn't get anywhere. When that happens and the man doesn't end up in a... passionate situation, he'll gripe that she left him with "blue balls" or to that effect.
